I know that running/walking is a potentially hazardous activity. I attest and verify that I am medically able and physically fit to participate in this event. I am aware that the COVID-19 pandemic creates additional risks of participation and I acknowledge that by participating, I risk exposure to the novel coronavirus and/or development of COVID-19. I acknowledge that I am voluntarily choosing to participate and have considered those risks. I expressly and specifically assume such risks associated with the novel coronavirus and/or COVID-19, including any and all risk of injury, harm, or loss that may incur as result of participating. By signing this document, I certify that (1) I have not traveled out of the country or been in close contact with anyone who has traveled out of the country within the past 14 days; (2) I have not had close contact or cared for someone diagnosed with COVID-19 within the last 14 days; (3) I have not experienced any cold or flu-like symptoms (cough, fever, headaches, muscle aches, rashes) in the last 14 days.I agree that I will follow the rules that Fleet Feet Syracuse mandates for participation including, but not limited to, rules related to social distancing and provided amenities. In consideration of your accepting this entry, I the undersigned intending to be legally bound, hereby for myself, executors and administrators, waive and release any and all claims I may have against Fleet Feet, its employees, volunteers, vendors, the Towns of East Syracuse and Dewitt their representatives, successors and assigns for any and all injuries suffered by me during participation in said training program. I hereby grant permission to Fleet Feet and any other sponsors of this program to use any record containing my likeness for any purpose whatsoever.
